1) If a boy completes an Outpost Adventure (an OA) on his own either as a Builder or as a Sentinel, then he earns the Outpost Adventure Patch and places it on the side of his uniform based upon his current rank - right side for Builders, left side for Sentinels.
2) If a Sentinel completes the Sentinel section of an Outpost Adventure that he had done as a Builder (which would be sometime later), he receives another Outpost Adventure Patch and places it on the Sentinel side, the left side, of his uniform. This is a different challenge for the boy and thus earns him the same Patch twice.
3) If a boy completes the Builder and Sentinel sections of an OA at the same time, then he earns an Honour Star as well as the Outpost Adventure Patch. However, this cannot happen if he has already earned the same OA Patch as either a Builder or as a Sentinel - there's no honour in completing the same work twice.
4) If a boy completes a Module with his Stockade, it will be a completely different experience. He earn the OA Patch, potentially for a third time, and places it on the side of his uniform based upon his current rank. This means that he could have the same Patch on the same side of his uniform twice. No problem.
5) If a boy completes a Module with his Stockade (which means that he has completed a number of the requirements in the Outpost Adventure as part of the Module) and earns the OA Patch, if he then completes all the requirements in the OA for both the Builder and the Sentinel within a few weeks of completing the Module, then he can earn an Honor Star.
6) While a Builder can move up and do the same Patch as a Sentinel, a Sentinel cannot go back and do the Builder section of an OA - he is too old and there is little challenge in his doing something for a younger boy.
